NOTICE OF PRACTICE CLOSURE
Dear Patient,
I am writing to inform you that MyHeartLife PLLC will permanently close effective November 30, 2026.
It has been my privilege to serve as your cardiologist and to provide direct cardiology care through MyHeartLife PLLC. I am sincerely grateful for the trust you have placed in me and in the practice. Caring for my patients has been an important and meaningful part of my professional life.
Continuing Your Cardiology Care
Because the practice is closing, I encourage you to establish care with another cardiologist or appropriate healthcare provider to ensure continuity of your medical care, particularly if you have ongoing cardiac conditions, medications, testing, or follow-up needs.
Please make arrangements for future appointments and prescription needs before the practice closes. MyHeartLife PLLC will no longer provide patient care after November 30, 2026.
Your Medical Records
Following the closure of MyHeartLife PLLC, your medical records will be maintained by our designated custodian of records, Desert River Solutions.
Beginning December 1, 2026, requests for copies or transfer of your medical records should be directed to Desert River Solutions.
